Prior to coming into our office for your eye examination, please print and complete our office patient information forms.  This will save you time during the check-in process.

You may arrive earlier than your appointment time to preselect your new eyewear. In the event that you need to be dilated, this enables you to make a confident choice in frames without your vision being compromised.
